AHCCCS is releasing time-limited directed payments as a way to incentivize individuals to enter or remain in the workforce. AHCCCS requires that all eligible providers complete this attestation form at the Tax Identification Number (TIN) level. Please note that providers who do not complete an attestation will not receive funds.
